Looking for someone to add two *required custom fields for me. first field is * IMEI: The first 15 digits. e.g. 111111111111119 (written above text box) with text box, this must have validation of at least 15 digits 2nd field is *EMAIL: Your unlock code will be sent to this address. with text box, this must have validation of at least 15 digits Both field must be required. 2nd field is to be placed at end of all the product options Please see example for text box [login to view URL] this is mine [login to view URL] VERY IMPORTANT. The information fields must be carried through the check out box so i am able to see the information on my end.
