Salesforce Lightning Component for Survey & Create Lead

Build a Lightning components with 8 questions and answers (toggle checkbox - Yes/No). Display "Score" based on the answers, each Yes has a weight-age of 2 or 1. If the score is greater than 7, then display Create Lead page. I have base component and custom lookup fields for Lead already. Survey Component: - Display list of questions and a toggle checkbox for each question with Yes/No as options First 4 questions as weight-age of 2 and other 4 questions has 1 - Open a Create Lead page once the survey is submitted and Display Score on it. Create Lead Page changes: - Check the attachment for current status of Create Lead page - Add date component for date two fields which should invoke native calendar on phone. - Add toggle checkbox component - Add Filter by RecordType logic on current Lookup Component Please mention the correct delivery timeline when you respond.
