Multiple choice printable forms

I am looking for a Joomla component that will allow me to set up multiple choice forms for my clients, when they have finished the choices on the form it will save the document into their account where they can print the result in a formatted form, the idea is to set up questions many of which will be multiple choice, depending on the choice the client makes it will pull the prefilled data relating to their choice and the other choices they make and then finally produce a document they can download which looks nice and is well paginated, certain elements the client will fill in such as address and some other areas, the idea is my clients should be able to fill in a few bits of information and run through some multiple choice questions the result of which will give them a document they can download.
