Contact Form

Need someone to build a good looking form. The data collected via the form needs to be pushed to Insightly CRM. The address bar should also search the address using google api's. Also the form needs to be a multi page with progress bar. Option 1 - Insightly can build a form using HTML but that needs to be modified to match with the site theme to make it look good.
