Add a non-working client authentication (username and password boxes) on home page

I just need a username and password box added to my homepage (with "Login" button right below) as depicted in attached pic (not slanted like pic). I need the login boxes to be directly to the right of my Wordmark at the top center of the page of Algo centrix dot com (with no spaces). They should be small, and match the height of wordmark. Again, the username and password do NOT need to be functional. When someone tries to login, there should just be a message below login say, "Login Failed". That's it. I will build what goes behind this authentication boxes in a few weeks from now.
