Interactive PDF

We are looking for someone who is competent in creating interactive PDFs. The project is a Member's Guide for a travel company. This is made up of approximately 50 pages. These have already been designed in Quark. Within this PDF there are several interactive booking forms, which the end user should be able to fill in and submit. This email then goes off to my client, whilst the end user should receive a message back on their screen saying the form has been sent. It is important that: * the info that is received by the client is readable * this can also be downloadable to PC so that the end user can view the PDF whether on line or off (but of course they will only ever be able to send the emailable forms if on-line).
