Need Expert on Google Sheets -- Re-Post

Small job - Need Expert on Google Sheets to advise me how to create and lock-down a sheet which will have multiple user inputs. I have an existing example which I want to replicate - this will be shared with the winning bidder. I have attached screenshot of existing Google Sheet - which seems to be locked as I can't show the headers or modify it - or replicate it - which is what I really want to do. Should be no more than a 10-20 min job for an expert who can explain what I need to do to replicate it.
