QR app needed

Hello, I need an App in IOS and Android that allows users to do the following: 1. Read an empty QR code an open a new page. 2. Then a new page appears were a blank fill-up form will be filled and associated it with that QR code. 3. Send that info to a webservice where the QR code and the info associated from the fill-up form will be stored as a line on a spreadsheet, wich it will have access for me, the Admin. 4. The digital number of the original QR code must be placed as a value with cell title "QR" on the spreadsheet with all the fields from the fill-up form wich will be around 10 fields, some will be mandatory and some won't. 5. All users of the App will be capable of read and create data on a fresh new QR code with they're smartphones, but, if a QR code is already associated with data on the webservice from another user it will let you edit it instead of showing you a totally new blank fill-up form as information on that QR has been already associated previously by another user.
