Custom Form with few pages and multi page

Hello I need a form like [login to view URL] the form needs to have the same fields but functions like uploading pictures and etc for example please fill out the form to see what happens i need the same exact thing for example if the person fills out application and picks married and 1 children it then gives another form for married and the two children after that it will show you upload pictures form where it will say upload picture for FIELD 1 and etc I need this form to be done in 2 days it is for a wordpress site i need all the info submitted to my email thank you
