Booklet Layout NEEDED ASAP! Experienced Adobe "In Design"

ASAP... We need a booklet layout In Design,(final print size 5.5" x 8.5"). The booklet must be laid out like a book (novel) with alternating page head graphics, an index, etc. The work is about 2000 words.
