Off line survey program

I am looking for some code that will administer an off line survey- either as a standalone program or maybe within excel. I want to create a True/False questionnaire and want to administer a number of short scales at one time (I am not certain of the exact wording yet, so I need to be able to customize the question text and number of items). After the person has completed the survey, I would like to run basic analyses on each scale and transform raw, summed scores into T-scores using means and SD’s I will input later. I’m not a programmer but I can edit formula if they are in VB where I can reason through the language and structure (especially if you can include a few notes). So, essentially, I need editable code for a survey (this one is true/false format but I might want to edit the structure in the future to have Likert-type response options – such as, 1 to 5-point agreement scales). I need editable code that compiles into a simple presentation of questions on the screen one at a time (from a list that I populate) and I will ask people to click a key- say the left mouse button for ‘true’ and the right mouse button for ‘false’ (or for Likert type surveys, I could use number keys 1-5). The person’s key press would then be captured and the next question presented on the screen. When the person is done with the survey, I would like to auto save his/her responses and then, later be able to sum groups of items, transform them into T-scores, and present the results in chart format (e.g., an excel line chart with each X axis mark indicating a subscale and the Y axis representing T-score elevations. I could imagine this working in excel or as a standalone VB program which could yield scores I easily move into excel in order to present them.
