Survey & Report system

Ok this is what i need... [login to view URL] IDEALLY.... I want my registered users to be able to request a new survey + set the total completions needed.. *REQUEST SURVEY 1.. Select number of completions needed (50, 75, 100, 125, 150) auto fill pharmacy name / info if possible *data will already be in the WP database *Request form complete.. Thank you, blah blah here is a link to your SURVEY website/survey/random-string AGAIN.. if possible = embed code to add it to their website (possible make a basic WP plugin to allow users to use shortcode on there WP site to add there survey [shortcode-survey 1].. but that could be discussed after)... NOTE... All users will have the same surveys.. E.G.. [login to view URL] The only change to each survey will be name / link details.. E.G Sandylight = USER INFO Admin will setup any new surveys, we do not need complicated editing systems, if its possible to simply... Replicate survey 1 and call it survey 2 . Even if this is done via hardcode changes.. We may in the future need to add / edit a question... at which point we are aware that maybe more difficult.. SURVEY FINISHED Ideally we would like a counter on the surveys so user can see how many have been completed, simple 05 | 50 or something... Once last one is completed e.g. 50x .. Thank you, our survey has now been completed (or something) .. This should then auto generate a PDF report similar to as attached, email it to the user.. IF POSSIBLE maybe add a link to the users account / page .. e.g... Click surveys page = Request new survey? + list previous survey results.. "username + survey name + date .pdf" NOTE: The site this will be used on isnt yet built (we are waiting to see costing of this, as currently we have a workable system via joomla... we just want to get away from joomla) IF SUCCESSFUL = 100% chance of ongoing work we are always looking for WORDPRESS experts.
