The call will being transfered from any extension to this IVR.
I need to build a post call survey to make customers 3 questions and save response (dtmf) into mysql.
Audios will be prerecorded audios.
Also need to associate to the same data: callerid, time, date.

I have similiar configurations for existing customers
where incoming or outgoing call gets transfered with a short code (*XX) code
based on the code the call gets transferred to IVR section.
The IVR section and short codes are done with PHP and MYSQL backend for reporting purposes
Having extensive experience in the field of Asterisk based systems and specially IVR products, I am the perfect candidate to accomplish this job.
