Web-based project survey tool

This is a web-based tool. It will have a responsive web form that consists of survey questions and possible answers (score 1 to 5 for each question). Once all answers are gathered, the tool will aggregate them across categories and show the final scores on a spider diagram or heat map. Each corner of the diagram represents one of the question categories. The tool needs to have the following features: - HTML5 responsive design (flat design, basic icons) - user account management - account security - secure storage of historical results - allow the creation of separate projects and phases in the projects, and then allow the user to run the survey for each phase allow the visual comparison of past results (e.g. on the same graph, to show the change over time) - allow setting of super user or admin These are the barebones features. I have mockups and wireframes to assist with the design (attached). I'm looking for someone who can help build this as a prototype on a small budget, then we can look at whether more features can be added. I own all rights to the code.
