Add a 'Sales Person Locator' for Wordpress site

We are building a Wordpress site and would like to add a page to find sales people for specific regions. We want our 'locator' to function like the one on this site: [login to view URL] So the user would select a country, then state/province, then the existing sales people in that area would be displayed below in a table. Project requirements are: - Must be able to manage the sales people from the Wordpress admin section. - Sales people must have the fields: Name, Address, City, State, Zip, website, email, phone, telephone, fax, Rep Type (FMI or HTI) - From the front-end the dropdown should have a search box like in the example - All Countries/states will be available when adding a new user in the back-end, but in the front-end only areas with at least one sales person will appear. - Ideally this will be an existing plugin that we install to the Wordpress site. If you know of a plugin let me know and I will purchase it. - Sometimes a country will not have states/provinces. In that case the system will just display the Sales Reps and not ask for a state/province. I can provide a list of sales reps to input into the system.
