Remove index.php from URL without errors

I need a plain URL. The URL contains [login to view URL], which needs to removed and work with out errors. We have tried modifying .htaccess and [login to view URL] files. we could get clean url but 404 error and looping redirect. So who can do this can message. Note: CMS installed is Open Journal Systems (Not wordpress)
