Build an ASP.net Intranet site for Windows Server 2012R2 and MS SQL 2012 - Lab Sample Tracking Log

I need an intranet site that logs stability chamber tests. There will be a main form (one) that has the sample id, product id, description, etc. that will have calculated fields such as the logged on Active Directory user, estimated end date, etc. There will be a subform (many) that records all tests that need to be done. The frequency is in months. The next test date is calculated by create date + frequency + extend days if necessary. All test will be inputed and next test dates and the est_end_date will be calculated from records above. I am attaching a screen mockup and an excel sheet that shows the date calculations. There will be email reminders that a test is ending in 3 days, 2 days, 1 day, and tests that have not been marked complete, if the test is extended more than 3 days than the emails will be nulled so they can repeat when the revised due date arrives.. If an end date is inputed in the main form then all test in the sub form will be marked complete. Access is determined by AD User and if there AD username is in a SQL table with their email address and employee ID. All data is the main form and sub form will have its data entered into an audit table that tracks inserts/updates and new/old information. We do not wish to use any other solutions other than ASP, .NET and Microsoft Windows IIS, IE11, Chrome. This project was 80% complete with a freelancer and then they asked for more money to complete, this is not acceptable and please be sure you are able to complete the project. Time is not a rush. Our availability for this is Mon-Fri 9am-4pm Mountain Standard Time (UTC -7) while also working on other items. Will respond as quickly as possible. I was completely surprised by the cancellation from the other freelancer, we were willing to work with them to complete the project. We do not have unreasonable expectations.
