Developing business company assesement to

We are in need to develop a tool which help us to assess our client companies. The tool will be used to assess a company from all angles of Managment, Startegy, Planing, Sales & Marketing, HR, IT, Legal. Later the same tool data will be used to compile a report telling about the current status of a company and suggestinh improvement actions and recomendation. The tool format can be suggested by freelancer or we can provide examples. An expected format can be an excel file with separate sheet for each angle (functions) of business with rows to list the assessment areas an columns to list the current level of the area. Each level will be having pre-written state description from which the assessor will select the releative level. At the end there will be a summaru sheet summarizing all assement relsut with a chat illustrating the result. An example of expected format is attached.
