Design facebook fanpage

here is my current facebook page [login to view URL] I would like it to be redesigned based with the unique and great look. Images, and video required could be taken from my webiste. I can create a subdomain, and provide ftp and mysql access if required. I would like to have great professional and innovative pages designed installed and fine tuned for fanpage, non page, profile images, etc. Also install necessary application to provide a great functionality including, having a contact form, review form, and a contest form, where in user can send in their recipe online. as you see here..
