Code UI (some design) order page of Lost Pet website

I need our service selection page (see attachment 2) redesigned to be intuitive and informative, and motivate the person to upgrade their pet alert to a paid package. Looking for similar to the provided mockup (see attachment 1), but there is more available to work with in the wordpress theme (font awesome, bootstrap, some styles and such). I will PM the original theme. Professionals in jQuery / CSS / HTML / Bootstrap ONLY! I would like the first part of the design to highlight the free services, and then below it should try to upsell the paid packages ("amber alert services"). There are 3 packages right now, but we may change to 4 or 5 even, so keep that in mind. The package selection area should be simple and condensed, so that the visitor can easily choose (on their mobile phone) what package they think will be best - as the slider moves the values are updated and price. Attachment 1 is a mockup I made, attachment 2 is how our package selection page looks now. Please no copy and paste bids - please send me 1 or two works samples, as I would like to look at your jQuery / CSS code. And please bid FAIR price so I actually look at your portfolio - there will be further work for you after this project.
