Creating / desinging small and professional templates for websites

For 3 new website i'm looking for somebody who create the up-site template. The distance are 990 x 190 px. I expect fom the provider that he create for each website 5 Raw proposals, each of which 1 is chosen, for the ultimate template. In the template is also the name and a logo...or what we can use for logo.
