Convert a Template to a Website

My company developed a number of truck driver training modules that run on a kiosk and are currently deployed in various sites around the country. I would like to take the current application that is run using a mix of flash AS3, AS2 and php and convert it to run completely online and be compatible with the 3 suggested platforms. A web component, an android component and an iOS component. Ideally all programming can be done in html5 to be compatible with all three platforms. The look and feel can remain the same but the questions can not be changed. All video files can be links to youtube content. Please add the following functionality: Log in capability which will correspond with data from a database. Ability for a new user to register and take the assessment. Log the completed test result as the user in a database with the corresponding test name, test score and location, time and data stamp Keep the same basic question functionality as is in the current test set up but optimize it for the new frame work. Project turn around time 2 - 3 weeks. All graphics, as3, as2, php files can be supplied upon request.
