Create simple intro and outro and edit existing cuts together for web explainer video

We need a simple intro and outro with our logo, and basic animation and transition music. We will want the files so that we can use it on an ongoing basis for our video blogs. We also want a 3-4 minute video edited together from the existing 10 files into one, with the intro and outro added. The timeframe is 5 days from acceptance of the job. Ongoing work is available if we are a good fit. Please provide examples of similar projects you have worked on. Include the word "star" in your submission to be considered. Please note that we will be away from the office until 3 June and will review all applications then.
