Add upload feature onto products on webiste - OSCOMMERCE

Certain products I sell on my website require customers to upload a picture file or provide specific information with the particular product they are buying. I need to be able to provide a means for them to do this and then append the picture or additional informatin on to the order they made in the admin back end area. * I should be able to manually assign each new product with this requirement and it should not let customers add the product to the cart without providing it. Only the products I choose should have this feature. Also I have attached a .GIF of a gift wrap present. I want to professionally add the words "Gift Wrapping Available" on to this picture. This should not just look like plan text, it should look like a professional picture and then please add this to my website with a page that gives customers information about the gift wrapping service. I also need my phone number 877 840-PINK added to the top of my website to the left of the mcafee secure seal, and the live chat link moved to replaced the 3 main flag pictures. This is all pretty easy stuff here.
