Hi, I have been deveoping websites with Joomla since 2008. Lately I have developed a lot of data oriented sites with a Joomla CCK (content construction kit) called Seblod. Seblod allows to extend the article manager and the user profiles. It integrates perfectly with Joomla (2.5 or 3.x) without hack thus making sure that upgrades are possible in the future. It's offers multiple languages compatibility (I understand Italian by the way). It also makes use of Joomla standard access rights configuration and allows to define different profiles (patient/physician for instance) with different access rights. It also keeps all versions of records even if they are updated. I took the example survey, Seblod could be configured to store the answers (with versionning) and give patients a report of their latest score as well as a history of their scores. I understand that you also want an appointnemnt system and the ability to save medical history information including files for X-Ray. All this can be handled with Seblod. It would be helpful if you could share with me the URL of your current site as well as the version of Joomla you are using (if older than 2.5 it would require an upgrade) and some indications of the volume you expect.
