Develop wordpress plugin - serial number entry page

I need a custom wordpress plugin written that allows authorized wordpress admin users to keep track of product serial numbers for each registered wordpress user. The plugin needs to do the following: 1. Upon plugin installation/activation, create the mysql ${wpdb->prefix}_serialtracker table to store the data. The table will consist of 3 fields, wordpress user ID, serialno (varchar 24), and identifier (varchar 48). 2. Inserts a section within the [login to view URL] screen called "Serial Number Management" where authorized admins can enter,view, or edit an arbitrary number of serial numbers & identifiers for a particular wordpress user. Each row of the table would contain 2 fields: "identifier" and "serial number" and the user should be able to add additional rows as needed. When updating, you must enforce requirement that both identifier and serial number are not null. 3. Have a separate page in the admin section that displays all serial numbers, & identifiers (and associated wordpress username and email address) and lets you sort by any of the fields. The above additions should appear only to wordpress users who have been given authority to do so in the user role editor, so a user role for this function needs to be created. Also, the plugin should be multi-site compatible and properly use $wpdb->prefix and other wordpress coding standards.
