ITSE.1407/COSC.1420: Intro to C++; Group Project Create a C++ application to have the given menu. To process each menu option you must use a function. Your program should store information from a patients’ records in an array of six patient records using patient structure. A flowchart is included which represents a cardiovascular disease assessment we use for each patient. For choice 10 create an array named counter(3) ( zero based) with counter(0) for smokers, counter(1) for HFD, and counter(2) for HBP. You may use the following sample persons.
