Auto Dial a User and Play back a message with asterisk and MS SQL

I have an inhouse Asterisk (trixbox 2.2 CE) phone system. I also have a 2003 Windows Enterprise web server with SQL 2005 installed running on it. I want a simple web form built that runs a simple stored procedure. The sql procedure has a known primary key (for example 4444) where it returns a single record of customer information First name, Last Name, Address, Phone number. This record is this passed to the Asterisk server where it auto dials the phone number and upon someone answer the phone it reads the data to the user. Values should be written back to the database if the phone was answered or not, time answered, and have a way to auto retry a certain number of times. I want documentation showing what change and software is setup on both servers to make this happen, along with any code and scrpt changes. I want to use this as a learning experience so I can continue to tweak these changes and further incorporate them into my application as needed. I would like to first see a working example on your system that I can test, and then I will provide access on my systems for you to install and get working. For payment, it must work on my system and I must have decent documentation showing what code and software installations were made. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
