Customized Data Collection Design

I am looking for the VERY BEST designer. I am creating a 5-6 page data collection tool. I have general layouts and ideas, The pages will be very similar to one another. I will hire someone else to do the database work. This is only a design job. I will attach an example of what I am looking for. I would like to open a private chat with the developer before we start so that the details of the job are very clear. 1. Each page will have some design work 2. A person will choose a box that describes them. (look at the example) When they click on the box a data collection box will slide down, or to the side, from the original box they clicked on. Data will be collected and they move to the next page. 3. I would like a progress bar will track their progress throught the 5-6 pages. 10% done....20% done... etc. If you think you truly are the best desginer please contact me.
