Add upload file function to website

I need a simple page where a customer/coach can click a browse button select a word, excel, or pdf file… upload it and the file will email to me. Right now with the attached form, the customer/coach has input this information that he/she has already typed once. So basically from the attached file I need everything (school name, coach name, comp name, comp date, phone, email and Confirmation or Cancellation). …and then if the coach states the competition made, then I need for them to attach their file with the teams that are competing. If it did not make, I will not be necessary to attach a file.
