Add a simple form after a Paypal buy now button is clicked

I have a page of various services and each service has a paypal buy now button. The new functionality I need is a popup form with 4 simple text fields (name, email, etc) to open when the paypal buttons are clicked, and before the buyer is brought to paypal. When they complete the popup form, they would click a 'continue' link and then they are sent to paypal as normal to process their payment. At this point I would get the info from the form in an email. I don't need a database. The form information needs to be sent AFTER the purchase has been processed at paypal, not before. Nothing terribly complicated. Budget $20.
