Create checkboxes with submit button and output choices to flat file

Given a flat file with names one bellow the other. You must create an output (html web page like preffered) which will display the names with a checkbox next to them and a submit button. When the user checks the names desired and clicks on the submit button it will outputs the checked names into another flat file.
