A simple login-secured php/mysql page with a database

a simple login-secured php/mysql page is to program for managing annually fees for a non-profit association. The whole should have a loginpage, a listview of the members, and a form to create/edit members. the member datas are: ID date of registration (this field should automatically create date for new members for old members it should be editabel) Name Surname Birthdate and place insurance policy number Name of father and mother adres 1 adres 2 phone number each member can have upto 10 relatives, but must not. if they've relatives then a separate input fields for each relative should appear with this fields: ID + number (eg if main user has ID 1001 then the first relative has to have 1001-1 name and surname birth date insurance policy number degree of relationship (dropdown with child, grandchild, spouse) Relatives which are childs and/or grandchilds can only be a relative up to their 18th birth then the system should give out an alert that they've to cancel. On each years 1st they the system should generate a list with the members and the total he have to pay. The total is to generate per input, this means there should be a field for the fee per person on the current year. In this field we've to put the total eg 10$ and the system should calculate. Eg if a person has 4 relatives and himself then the system should calculate for him 50$. Afterwards the system should generate a field for the current year where a checkbox is to seen for paid or not, the past years should also be seen. if possible the systeme should print on click for each member a bill with fixed positions. Estimated programing time is about 15-20 hrs for a skilled programmer with min. 3 years experience. Skill requirements: good knowledge of PHP, mySQL it's an urgent project so pls. don't bid if you haven't got enough time Project rules: 1- Payment would be escrowed fully at the begin, after finishing the whole project i'll release pls. dont ask during your work for release 2- a testserver would be provide 3- The given and accepted time is the dateline if the candidate couldn't do the job in this time and don't inform me in-time the job would be cancel 4- i would be informed frequently about the progress 5- It's not allowed publishing this work in private portfolio or elsewhere and sharing this 6- All parts of programming should be commented and a little tutorial/documentation should be written afterwards
