Data Entry Ajax Form

Data Entry form using Ajax Form will need to allow for single item association to multiple brands, years and year limited SKU's. Interested in single page/form with popup/overlay for large list of options to be selected. Upper Part of Page: SKU with multiple fields and image(s) upload Lower Part of Page: Association: Multiple brands Selection Popup, Next: Filtered Multiple Year section popup grouped by brands, then filtered Model popup grouped by brand. The backend is not included in this project, but query should function with flat file data if possible. Willing to consider better ideas
