Add New Page Template to Wordpress Theme

Hello, I am looking for a coder to add a new "Sales Page" page template to an existing Wordpress Theme. I have attached a PSD mockup of what I need the page template to look like called "BigVideo.psd. I have also attached the theme I need the new page template added to. There are a few special features I need added to this page template: 1. I have attached two videos that I need to play on the page. The first video will play at the top of the page where I have indicated in the mockup. The second video will open in a lightbox when someone clicks the "Watch Conversion Robot in Action" message. The videos will be encoded using JW Player, which I have bought a license for. 2. A simple JavaScript popup that opens if the visitor tries to leave the page. An example is available here: [login to view URL] 3. FAQ and Disclaimer information (attached) open in lightboxes from the links in the footer area of my mockup.
