Develop a Web Application

I need a web application designed that will allow a user to update two fields then have a soap request submitted to a server with the updated information Requirements: Web application with two fields named... 1. userid 2. subscribeCallingSearchSpaceName a. ATLANTA_195_DEVICE_CSS b. GRANDRAPIDS_135_DEVICE_CSS c. ITA_DEVICE_CSS The first field 'userid' will be a Text Input field. The user will enter their User ID The second field 'subscribeCallingSearchSpaceName' will be a Drop Down Box with three different choices: ATLANTA_195_DEVICE_CSS GRANDRAPIDS_135__DEVICE_CSS ITA_DEVICE_CSS FOR EXAMPLE...
