Create a login page

I am looking for someone to create a iPhone log in page. The page will be on the internet and will just be viewable from the iPhone... So it will just be a html page with a url... if you type the url in the web browser it will show you the login page.... so when the url is put in the browser of an iPhone or iPad they will see a log in page... its simple stuff if you have any questions please ask I have the design... I have the code for the username and password... I just need some one to create the design into a working page and place the code to the two text fields so when some one entered the username and password it will show and then login in to where it needs to log in. The design must work with all iPhones and iPads and look good in design and function well You must have worked with this sort of thing before. Its a really easy project hence the price
