I think your budget is too small for the complexity that you have asked so I have put in the lowest bid for which I can do this. Thank you for the specification document - from it I was able to deduce that what you need is a form that allows you to choose up to ten medication types, for left | right | both and with a frequency and start selector. You would then press a "Create Report" button that would make an Excel tab with the data on for you to check and to save it to disk as a PDF; You can then PRINT the report from the app. You would have to create a list of medications to drive the drop-down but it will be quick and easy to use and generate a quality report you can give to your patients. I would also suggest the report should have your contact details on it, with a telephone number, to make it look professional. I think the best way would be to create this in access or excel, using dropdown lists for medication, frequency, left or right eye and start & end date as variables. this can then be output into a nice format for the patient to take home and use.
