Design (Aesthetic &Logical) & Creation of a form in .DOC,.XLS & PDF

We are looking for a form design specialist to create a professional looking form for us in MS Word, MS Excel and Adobe Acrobat Form formats. We would provide a draft format of the form in .DOC version as well as some notes as to which fields we would like to be included within the form. The consultant would first need to rationalize and group logically similar information together - such as "personal information", "Contact Details" etc. The consultant would create a list of field names and field definitions (type of input - eg. alpha numeric, numeric etc. and number of characters) The consultant would need them to create an effective layout which maximizes space utilization. The form would then be created and sent to us as a PDF. We would review the form and send back comments by marking up the printout of the PDF and sending back the scanned copy of the marked up printout. The consultant would revise the form as per our direction and then resend another PDF which would effectively be the final version. Once approved the consultant would send us PDF FORM, EXCEL and WORD FORM source files. The form needs to be optimized to allow the printed version to be filled out by hand. Further, the word and pdf versions should have proper field definitions.
