PROJECT WILL NEED TO BE COMPLETED WITHIN A COUPLE OF HOURS. Project will be awarded to the first freelancer capable of completing today. I need an electronic form built to capture some information and accept payment via Paypal. The details provided are purely for content requirements only. Form will need to be designed to a professional level. Individual submission are to be emailed to us and full report of submissions available in a usable format such as exel. Form will need to be hosted and links provided. Prefer if any small edits can be done by us live online without bothering the freelancer again so a platform allowing this would be preferred.
