Create wizard based on a mock up - repost

We need a simple wizard with four steps in HTML in combination with jQuery. I have made an mock up and i want
the exact mock up to be made.
The user starts with step 1. and clicks on one of the icons in the white field (see example 1).
When the user clicks on the three little dots, called 'overige', then more icons shows and a back button appears
See example two. As you can see, this is still part of step 1 (stap 1).
If the user clicks on an other icon than the 'overig'one, then the wizard will go to step two,

On this step the user can slide the slider from left to right. There need to be six steps.
From left to right:

For example the user slides to the right and he will see the next example See example four
As you can see, some text appears and the button next. When the user clicks on next he will
go to the next step, see example 5
This step is exactly similair to step 2 (only we ask for gebruikers (users) instead of medewerkers).
Everything works the same as on step 2.
Step 4 is the last step, the user sees the choices he made and fills in some basic information, see example 6. As soon
as the user hits Send, then a thank you note needs to appear and the wizard is over.
Other things to make
I want to store every step of the user in a database/file. So when a user starts the wizard,
and he stops at step 3, i want to see this:

As you can see, 2 persons finished the wizard and 3 person did not finish the wizard and stopped (1 of them
even did not finish step 1)
Thats it, not to difficult, so when can you deliver it?
