Simple Data Order Form: Write an iPad application (Possibly Filemaker Go)

We will be taking our store single page order form to a iPad app and have the data on a cloud server. We are a small multi store operator and need all stores to be able to access orders. We require only one login and password for now, though it must work at all stores, or each store can have their own if easier. We want to be able to the have the following fields:
Customer Name
Tel#
Order Date and time (Automatically created)
Person taking order (List related to each location that we can update on a Mac)
Date To Pick Up Order
Store Location (Multiple Choice)
Pick Up Date and Time
Item Selection if Cake and qty to order.
If a Cake field for size
If cake field for icing
If cake filed for Border
if Cake filed for writing
If cake field of decorating.
If cake writing color
If cake border color
If cake Icing color
If Cake Fondant Selections
If cake Extras Selection (Radio Buttons)
Item Selection if Pie and QTY
Item Selection if Cupcake and QTY
Item Selection if Cookies and QTY
Item Selection if Other and QTY (With option to type in other item)

When the order is submitted we want to print a label with the order details and a copy of the to be emailed to the customer with the order details, emphasis on where to pick up, when and our company information.
We want a dashboard which is the default page the store level staff can access and choose the following:
•Create New order
•Look Up an existing order (At this location or Any location)
•Print a label by choosing the order (containing order details). We will use a LabelWriter printer with wireless print dongle.
•Print a copy of the order for the baker.
•Look up orders by a specific date and allow these to be printed in a list format and detail format.
• Show all orders for a specific store.
Software must be an APP on the ipad, we do not allow browser use on our iPads. Safari is disabled.
Our deadline to be finished and in BETA is Tuesday December 8, 2015
