First please check the link above. I'd like someone to reproduce this form with original code and not by any site soaker for example.
I need the form customizable for prize (I understand html and js)
Keep in mind that for every division in the division tag boosting the prizes between divisions increase.
I need a form consisting out of the three : Win Boost, Division Boost, Placement matches. The looks should match the forms on the website.
PAyment is only possible via paypal so i dont need all the special payment methods which are shown on the website.
Its important that the customer gets another form before he gets redirected which looks good. The form is for information like name email and stuff like this.
