Custom PHP script for [login to view URL] / chat

Hi

I am looking to create a simple PHP page that allows one to:

1) Provide [url removed, login to view] url (see [url removed, login to view] as they do a good job with that) to embed on the page

2) A simple easy chat box like on [url removed, login to view]

3) Optin form / html code

Please see the attached image

Need to be able to start immediately
