PHP GUI to modify records of a table, Smarty required (ref:MoP)(repost) (1165171)

We need a 1 page PHP module, that will pick a record from a MySQL table to modify a single field (from user input)

All include files necessary are in the template php which is attached.

So you do not need to code for login, user rights.

Use of PHP/Smarty is a must. It is already installed and tested on our server.

The details of the project (db scheme, analysis) are in the attached document. SQL script will be provided.
