Simple MySQL Database Needed for Insurance Leads Business We need a simple database loaded and operating using MySQL and PHP. It will be fed from the [url removed, login to view] page of HelloLeads.com. Open source processes must be used so no license will be required. The current form script is a simple [url removed, login to view] script. Your roll will be to create a simple database that will collect the data and load it into the backend. The dB script is to be provided by you. The current business model collects generic data and credit card data in separate forms. The form need functioning CAPTCHA. We need to include all this data in one database. The backend should have the ability to sort the information by each field, such as name, company name, state, zip, etc, as well as be exportable in a comma separated file. The backend should have the option to send email notifications to the admin email address. It would be ideal to view a similar working system the bidder has developed before the project is authorized, but not required. 100% payment will be made upon project completion. Provider agrees to provide all source code and bug-fixes for 30 days. Script must not require any external software. Buyer will pay for SSL certificate. Provider will install it.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):
a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
