Build an app to auto populate data on a third party website

I am a doctor who repeatedly enters patient data onto a free to use website calculator ([login to view URL]). The data come from calculations made in my office resulting in a printout (attached).
I would like to write an app that:
1. takes a photo of the printout
2. using OCR is able to digitise the data
3. auto-populates the fields at [login to view URL] In total there are 11 fields to populate
