App for home Inspection --> Calculation Excel Workbook

Our company field reps need an Android and possibly iPhone/iPad application that will present basic questions about the condition of a home being inspected. Answers will be primarily numeric with an option to add a comment to select questions.
We want the results to be transposed / mapped to our preformated Excel worksheet for further analysis. The option to Save, Send to Email and Send to Dropbox should be presented at the end of the questionnaire.
When a question is answered during the inspection, we need a calculation to be performed and displayed next to the question. Our worksheet will calculate the result, but we need your help to bring that data back to the Android app.
Our predefined workbook is often changed annually, so the option to insert / alter questions should be considered when developing. We are open to some development ideas, but have a specific outcome in mind.
