write html to mark checked supplier links in wordpress

Hi
I have the plugin optinmonster on my wordpress site. I use it to display an empty lightbox popup.
You should provide me with the code to fill this empty space. I'm looking for a way to show a list of suppliers of various services. Each supplier on its own row. (name, thumbnail and linkbutton to their website) Users should be invited to "check" the suppliers website by clicking on button.
As soon as they click the button a few things should happen:
1 a new window should open (website of supplier), while the original popup remains open
2 the checkbutton should change color
3 the checkbutton should hide the text of the button, and replace it with a check mark.
See the example in attachment.
As users progress, more and more links will be checkmarked.
You should provide me with all the html, css, (images?) and scripts in order to get this working. Use your own demo content and give an example for 5 suppliers (5 rows). It should work on all major browsers and also on mobile devices.
