Redesign of 7 excel sheets in word or indesign

I have an excel sheet that is made up of 7 forms. These need to be redesigned in either word or indesign so that each form has a similar design theme taking the first excel worksheet as the closest example.

Once designed, these will then be:

1.) Created into a PDF Form

2.) Translated into 3 other languages (translations provided)

Therefore, please ensure that the design is easily editable.

Please also advise if your bid includes the changes into the other translations and conversion into a PDF form, or just the design for the 7 forms.

Final output should be A4 Landscape or Portrait
